One of an unknown amount of clones, Helena is unique in that she's also a twin. Whatever happened in the womb with her and Sarah has also allowed them to avoid the natural sterilization of their kind, resulting in Helena having one bio niece and being pregnant with her own child. She's also batshit crazy from having been brought up as an assassin of her kind.
